The Guide to Program Implementation aligns with the State Program Monitoring Instrument and is designed to help guide agencies to plan for success. It reflects an outline of various planned activities to determine how your agency might monitor & evaluate its EESD center-based & family child care home education network programs.

Data Collection 

Center-Based & Family Child Care Home Education Networks collect data using the Program Review Instrument, Desired Results Parent Survey, Desired Results Developmental Profile & Environment Rating Scale to complete the self-evaluation.

Alternative Payment & Resource & Referral Programs collect data using the Program Review Instrument & Desired Results Parent Survey to complete the self-evaluation.

Data Analysis, Plan for Program Improvement & Ongoing Monitoring

Contractors are required to analyze findings from the self-evaluation & develop both a written list of tasks to modify the program for all areas needing improvement & procedures for ongoing monitoring to assure that areas that are satisfactory continue to meet standards.

Communicate Plan

Contractors must communicate the results & plan with:

  • Parents

  • Staff

  • Board

Maintain proof of plan communication such as parent, staff & board meeting agenda & notes

Follow-up & Reflect

Periodically programs must review the status of the plans.

Record follow-up dates, modifications or changes needed to complete the action steps & expected completion dates.

Reflect on the results & record progress or changes made to the key findings.

Reporting

Contractors must submit the Program Self-
Evaluation (PSE) reports to the California Department of Education by June 1 of each year either electronically or through a hard copy.

Forms EESD 4000A & EESD 4000B are used to fulfill the submission requirement for the PSE. Included below is the link to instructions for completing these forms.
